Health Spa Information Update

As a current or former health spa owner or manager, you received a letter asking for an update of your business information from the date your contract was approved for compliance with state law.  Please print out and submit the Change of Ownership form if the business has since changed ownership.

If the health spa has been sold and you are no longer associated with it, complete items 1 through 8, sign the form and have it notarized.

If the business has been sold but you are still presently involved with it, please complete the entire form.

Send this form to our office, along with the current membership contract, if applicable, and the information verification sheet you received (this latter document does not need to be filled out but will help us match your information with the correct file).  Thank you for your assistance.
